ESXi 5.x host fails to reboot after installation with the error: Not a VMware boot bank. No hypervisor found

Issue/Introduction
Symptoms:
- After successfully installing an ESXi 5.x host, the host fails to reboot.
- You see the error:
BANK5: not a VMware boot bank
BANK6: not a VMware boot bank
No hypervisor found.
Environment
VMware vSphere ESXi 5.1
VMware vSphere ESXi 5.5
VMware vSphere ESXi 5.0
VMware vSphere ESXi 5.5
VMware vSphere ESXi 5.0
Cause
This issue occurs due to a prior partition table present in the local datastore.
Resolution
To resolve this issue, use a Live Linux CD of your choice. Relabel the local datastore and create a partition table with one partition. Then re-install ESXi.
