ESX Server 2.5.4 Installation Fails when USB 2.0 is Enabled in the BIOS (8749435)

Details

When attempting to install ESX Server 2.5.4 with a Hi-Speed USB CD-ROM drive on a server where the BIOS supports USB 2.0, the following error message displays and installation fails:
 
failed to read /modules/module_info
install exited abnormally

Solution

Disable USB 2.0 in the server's BIOS setup.
