Solution

Summaries and Symptoms

Installation of ESX 4.0 ESX400-201112401 fails on some x86 64-bit machines that use physical destination mode regardless of number of processors/cluster present with the following error message:

MP-BIOS bug : 8254 timer not connected to IO-APIC

Kernel panic - not syncing: IO-APIC + timer doesn't work! Try using the 'noapic' kernel parameter

The APIC (Advanced Programmable Interrupt Controller) delivery mode is unchecked in the Advanced Configuration and Power Interface (ACPI) Fixed ACPI Description Table (FADT), which causes ESX to set the APIC to a flat mode. APIC set to a flat mode does not work on some x86 64-bit machines that use physical destination mode.
