ESXi 5.0 async QLogic driver 911.k1.1-9vmw fails on 8G fiber channel IBM Mezzanine Card

After installing the the async driver provided by QLogic, 911.k1.1-9vmw, the firmware on the 8G IBM Mezzanine Card or IBM 44X1945 QLogic 8Gb FC Expansion Card fails.
The fiber channel cards no longer show in the storage adapters list.
Your system is unusable as no shared storage can be accessed.
The /var/log/vmkernel.log and /var/log/boot.gz log files contain entries similar to:

To resolve this issue, perform one of these options:

If you did not make any changes to your system after installing the 911.k1.1-9vmw driver, you can revert to the driver provided with ESXi, qla2xxx version 901.k1.1-14vmw. Press Shift+R when rebooting to access the alternate bootbank that contains the ESXi inbox driver.

Note: If you have made additional changes to your system after installing the 911.k1.1-9vmw driver, you must reinstall ESXi to resolve this issue.
Re-install ESXi.

This driver works for other cards.
