ESXi 5.0 async QLogic driver 911.k1.1-9vmw fails on 8G fiber channel IBM Mezzanine Card
Symptoms
- After installing the the async driver provided by QLogic, 911.k1.1-9vmw, the firmware on the 8G IBM Mezzanine Card or IBM 44X1945 QLogic 8Gb FC Expansion Card fails.
- The fiber channel cards no longer show in the storage adapters list.
- Your system is unusable as no shared storage can be accessed.
- The
/var/log/vmkernel.logand /var/log/boot.gz log files contain entries similar to:

Resolution
To resolve this issue, perform one of these options:
- If you did not make any changes to your system after installing
the 911.k1.1-9vmw
driver, you can revert to the driver provided with ESXi,
qla2xxx version 901.k1.1-14vmw. Press Shift+R when rebooting to access the alternate bootbank that contains the ESXi inbox driver.
Note: If you have made additional changes to your system after installing the 911.k1.1-9vmw driver, you must reinstall ESXi to resolve this issue.
- Re-install ESXi.
