Exceeding Host Bus Adapter Limit While Loading Driver Causes ESX Server Crash
Details
An ESX Server host fails to boot in the following scenarios:

ESX Server is configured with more than 16 storage HBAs (the HBAs could be SAS, SCSI, or Fibre Channel, or a combination of these).

One or more of the storage HBAs is an Emulex Fibre Channel adapter (during boot up, the ESX Server host crashes when the Emulex driver is being loaded).
Solution
ESX Server supports a maximum of 16 SCSI adapter ports. As a workaround for versions of ESX Server prior to 3.0.2, reduce the number of total storage HBAs to 16 or fewer.
This issue is fixed in ESX Server 3.0.2 and higher versions for both lpfcdd_732 and lpfcdd_7xx drivers.
