ESX Server 3i version 3.5 occasionally fails to reboot the server when a reboot is issued (1009394)

Details

On ESX server 3i version 3.5, the system might fail to reboot. This occurs when the following Sysalert warning is observed in vmkernel logs:
 
ALERT: UserCartel: 993: bootstrap/init UW died

Solution

This issue is resolved in ESX server 3i version 3.5 Patch ESXe350-200904401-I-SG.
For prior ESX server 3i version 3.5 versions, no workaround exists for this issue.
