SCSI: device set offline - command error recovery failed

Details

  • You may also see errors similar to the following displayed in red on the console:

    • 82:15:58:54.825 cpu0:1024 VMNIX : scsi : device set offline -command error recovery failed : host 0 channel 0 id 0 lun 0
    • 82:15:58:55:603 cpu0:1024 VMNIX : journal commit i/o error

  • Errors such as i/o error dev 08:02, sector 4397696 may also be displayed.
  • Service Console may become inaccessible.
  • When you attempt to log in you may see errors similar to:

    • INIT: cannot execute "/sbin/mingetty"
    • INIT: Id "3" respawning too fast: disabled for 5 minutes

Solution

A reboot of the server may resolve the issue and allow access to the service console. However, this may be a sign of a problem with your storage. Contact your vendor.
 
If the errors reoccur after a reboot, it indicates an issue with the disks or SCSI cables. Contact your vendor for support.
 
Note: IBM has a patch to resolve the issue if you are using IBM 3650 with SAS. Contact IBM to get the patch. For more information, see VMware ESX Server fails with journal commit I/O errors - IBM System x.
